Covenant HealthCare utilizes care paths, pre-printed orders, cardiology-specific discharge instructions, patient education materials, and outcome measures for open heart surgery patients. Each record is reviewed and elements abstracted into an STS database for national benchmarks. Meetings are held to review findings.

Percutaneous Coronary Intervention

Covenant HealthCare utilizes a care path, pre-printed orders, cardiology-specific discharge instructions, educational materials, and outcome measures. Each record is also reviewed and elements abstracted into the STS database for national benchmarks. Meetings are held to review findings.

Abdominal Aortic Aneurysm Repair

The development of a care path and other supporting documents and measurements has been approved and is in the planning stages.

Carotid Endarterectomy Surgery

Covenant HealthCare has a care path, pre-printed orders, educational documents and outcome measures.

Covenant HealthCare utilizes care paths and other supporting documents, and has outcomes measures.

Intensive Care Unit Physician Staffing (IPS)

Covenant HealthCare participates in the Keystone ICU project with MHA and John Hopkins. This includes improving the culture of safety, daily goals, intensivist rounding daily, ventilator care and prevention of pneumonia, prevention of blood stream infections, palliative care, etc. Covenant has been recognized for the accomplishments as part of this project.

Healthcare Failure Mode and Effect Analysis

HFMEA is utilized as a tool for safety, patient safety, and 6 sigma teams.
