Hospital Comments

Open Heart Surgery

Participating in Keystone ICU- ventilator weaning protocol. Limiting prophylactic antibiotic treatment to 48 hours for open heart patients. Implementing recommended treatment to decrease incident of atrial fibrillation post-open heart surgery.

Percutaneous Coronary Intervention

PCI-1 data is through 1st quarter 2006. Implemented STEMI Protocol. Empowered Emergency Room physicians to read & diagnose the 12-Lead EKG and activate the on-call team to streamline the process. Decreased door to EKG time. On-call team is on a global pager. End of day QA check of available equipment ready to use in Cath Lab. Working with Wayne County to pursue wireless transmission of EKG from ambulance to our Emergency Room. Appropriated parking spaces in front of the hospital for on-call team parking to decrease their time to get into hospital. Hospital president mandated the use of AMI order sets. Posted signs in elevators, physician lounges, and nursing units on requirements for discharge instructions.

Low Birthweight Infants and Infants with Congenital Anomalies

We participate in and have access to Neonatal NICU Network at the NICHD.

General Comments on Patient Safety Activities

FMEA's:Electronic Medical Record Transition in the Cath Lab and Medication Error Prevention with PCA pumps. Medication Reconciliation Process, autofaxing of radiology reports to physician offices, and electronic admission notices sent to attending physicians. Deleted medication range orders, created Diabetes and Radiocontrast-Induced Nephropathy standard order set. Executive Walk Rounds, Environment of Care Rounds,web-based incident reporting system, Keystone ICU Project, Infant Security/Abduction Prevention, Partnering for Patient Safety Newsletter, Look alike-sound alike medication posters, post-op follow-up phone calls, Fall Prevention Plan, Core Measures PI- AMI, Heart Failure, Pneumonia, SIP/SCIP. Committees: Leadership Performance Improvement and Medical Safety, Medication Safety, Medical Safety, and Emergency Management.
