General Comments on Patient Safety Activities

Paul Oliver participates with the Michigan CAH Quality Network to collect data and comparatively analyze data based on the core functions of the small rural hospital. These include Transfers, Pneumonia Transfers, Treatment of AMI and Inpatient Pneumonia, which is used to participate with the CMS data base through Q-Net. All department have individual PI measures based on process that need improved or monitored. Standard Outcome Measures Include: Inpatient Mortality Admission for adverse results following outpatient management Returns to the MPR/OR readmission Rate Critical Access Hospital 96 hour requirement Patient Safety Initiatives Patient Identification Medication Safety/Medical Reconciliation Standardized Abbreviations Developing process for better "hand off" communications and inproving communication among caregivers Implementation of CDC recommended hand hygiene guidelines Skin care process to decrease health care-associated pressure ulcers.
