Yes we have used the tool kit and have CCU nursing staff leading this initiative. We have also employed a full time intensivist who is certified in critical care medicine and has challeged nursing staff to become ICU certified also. The kit has given us information on certification locations and dates etc.

Healthcare Failure Mode and Effect Analysis

Although Healthcare may be new to the terminology to our hospital, most managers are familar with the "FMECA" process and some participate in one at least annually.

Root Cause Analisys

The leadership of this facility are committed to process improvement and do not use RCA in a punitive nature but as an opportunity to prevent occurrances from happening again. It is seen as a learning tool and we are encouraged to use it as often as need

General Comments on Patient Safety Activities

Although we are a small critical access hospital we have committed to participate in several national best practice initiatives such as; SAVE 100k LIVES Campaign, National Quality Forum Best Practices, JCAHO National Patient Safety Goals, ISMP, MICAH, VHA, MHA patient safety initiatives. We have an active patient safety committee that meets monthly and a safety committee that meets monthly. The leadership and staff share a commitment to providing the best care possible to our patients. We have initiated the Eden Philosophy in our SNF and are initiating the Planetree Philosophy for the hospital demonstrating a commitment to provide our patients with an exceptional hospital stay or procedure.
