The Michigan Health and Safety Coalition (MH&SC) is a collaborative quality improvement effort focused on improving patient safety in Michigan. MH&SC was first organized in 2000, following the release of the Institute of Medicine (IOM) report titled, “To Err is Human”. In 2006, MH&SC incorporated in Michigan and received approval from the IRS as a 501(c)(3) non-profit organization.
Leapfrog Hospital Survey (Version 5.1)
The Leapfrog Group
has closed their 2008 survey as of March 27, 2009 and is currently
reporting 2008 results on their Web site (leapfroggroup.org).
The newly revised 2009 Leapfrog Hospital
Survey (Version 5.1) was launched oon April 2 with a deadline of
June 30, 2009. First results will be reported at the end of
July. The survey will be open throughout the year for updates
and revisions.
