2004 Michigan Health and Safety Coalition (MH&SC) Joint Hospital Survey

The 2004 MH&SC Joint Hospital Survey will be launched on August 30, 2004. This year's survey continues the joint effort between the Michigan Health and Safety Coalition (MH&SC) and The Leapfrog Group (LG). The LG is a national non-profit organization representing purchasers. Participation in the survey is voluntary but hospitals have been overwhelmingly supportive of the effort in the previous two years. Your hospital MUST complete the survey by October 1, 2004 for your results to be included in the MH&SC reports that will be provided to hospitals, health plans and consumers in December 2004.

A copy of the 2004 MH&C Joint Hospital survey is available in PDF format below. Hospital CEOs were sent detailed information about this year's survey, including the security codes needed to access the on-line survey, on July 29. During September hospitals throughout Michigan are asked to complete the on-line survey for the following areas of care:

Open Heart Surgery
Coronary Artery Bypass Surgery
Percutaneous Coronary Intervention
Abdominal Aortic Aneurysm Repair
Carotid Endarterectomy Surgery
Pancreatic Resection
Esophagectomy for Cancer
Low Birthweight Infants and Infants with Congenital Anomalies in NICUs
Intensive Care Unit Physician Staffing
Computerized Physician Order Entry

New this year: Additional survey questions related to the National Quality Forum (NQF) Safe Practices

In 2004, the national Leapfrog Hospital Quality and Patient Safety survey has added a survey section devoted to remaining safety practices included in the National Quality Forum Safe Practices for Better Healthcare: A Consensus Report, released in May 2003. A PDF version of this survey section is posted on this page. MH&SC asks Michigan hospitals to complete this additional survey; a link to this survey will be embedded in the joint hospital survey. Survey results will be posted on the Leapfrog Group's Web site at http://www.leapfroggroup.org. MH&SC will use results for these other Safe Practices to implement improvement efforts, but does not plan to report results on its Web site this year.

MH&SC and The Leapfrog Groups suggest that hospitals order a copy of the National Quality Forum Safe Practices for Better Healthcare: A Consensus Report (May 2003) in advance, given the one-month time window for MH&SC survey submissions.
